SERVICE OFFER:

Our Stroke Recovery Service is here for stroke survivors, their families and carers to ensure they have the support they need to rebuild their lives post-stroke and into the longer-term. The impact of stroke can last a lifetime, affecting a person’s physical health, emotional wellbeing, family and finances.  

The Stroke Recovery Service makes sure that people affected by stroke have their needs placed at the centre of their recovery journey. Working in collaboration with others across the stroke pathway and within the wider community, we help ensure people have an integrated and seamless experience of care and that they receive the support they need, when and where they need it.  

Upon referral to the service, we work alongside stroke survivors, their families and carers to develop and deliver a personalised stroke support plan. This is based on what they tell us is important to them, including secondary prevention, emotional needs, and their goals.  

In addition we support stroke survivors and carers affected by aphasia (speech and language difficulties)

The service also provides high quality personalised information and tools for individuals to support recovery and life after stroke.  

REFERRAL CRITERIA:

The service will receive referrals for all those affected by stroke who are registered with either a Norfolk or Waveney GP and over the age of 18.

Service Offer:
We offer free initial advice with a family solicitor on matters such as divorce and separation, child contact, and domestic abuse.  We also offer help with court paperwork and support for people who are attending family court hearings without a lawyer.

Referral Criteria:
Our service is open to anyone who lives in Norfolk, or whose family case is being heard in a Norfolk court. We particularly welcome clients who may not otherwise be able to access legal advice.
We need to do a conflict of interest check before we can give ANY family law advice. Please include in your referral the name and date of birth of the other party involved in the case (ie the client’s ex-partner/perpetrator of abuse/other parent of the children, etc). This is ESSENTIAL for all family referrals!
