Social Prescribing – Norwich & Broadland

 The Living Well Team at Norwich and Broadland offer support to people who are experiencing either physical or mental health problems which are being caused or exacerbated by social circumstances and who may, therefore, be frequently attending primary or secondary health services. Advice, support and guidance is available in relation to a number of issues including budgeting, housing, social isolation, family and relationships, care and carers, consumer and welfare benefits. We are not able to provide debt or legal advice.

Eligibility

The service is available for any adult over 18 with no upper age limit.

Clients need to be registered to a Norwich GP surgery or, if not registered to a surgery, living in the Norwich area. Please see the note below.

People who are likely to benefit from the service include those who are experiencing, or at risk of experiencing, difficulties with:

  • > Budgeting
  • > Employment
  • > Benefits
  • > Family & Relationships
  • > Managing a health condition
  • > Housing
  • > Caring responsibilities
  • > Low mood or low level anxiety
  • > Loneliness and isolation

PLEASE NOTE

Norwich and Broadland Social Prescribing can only accept referrals for clients registered with GP practices in the Norwich Primary Care Network (PCN).  Many Broadland practices are NOT in the Norwich PCN.  Please feel free to ask before referring so as to avoid having your referral declined.  livingwellworkers@ncab.org.uk. If referring into the Norwich and Broadland Social Prescribing Project can we ask, where possible, the importance of obtaining an email address to enable a quicker contact due to delays with Royal Mail over the festive period.
