Social Prescribing – Norwich & Broadland

SERVICE OFFER:
We offer support to people who are experiencing either physical or mental health problems which are being caused or exacerbated by social circumstances and who may, therefore, be frequently attending primary or secondary health services. Advice, support and guidance is available in relation to a number of issues including budgeting, housing, social isolation, family and relationships, care and carers, consumer and welfare benefits. We are not able to provide debt or legal advice.

CRITERIA:
Our service is open to anybody aged 18 or over and who is registered to a GP in the Norwich Primary Care Network. Please note that not all Broadland practices are in the Norwich PCN. To find out more please e-mail livingwellworkers@ncab.org.uk

CRITERIA: Support can be provided to residents of Norfolk and Waveney who are 18 or over. We cannot accept requests for personal care, respite or support for those with complex needs.

We offer support to families and individuals who are facing mentally and financially challenging situations. We only operate in Norwich and the immediate local area.
